Release Forms

I have done a search on here, and I haven't really found what I'm looking for, same via google, so if you can help me or even point me in the right direction that would be fantastic.

I want some type of release from that my clients to sign to give me permission to use the images in advertising, and on my website/facebook page (for my business).

If I can't find what I want, what is the best thing to say? and then I'd be happy to type one up.

If you can help me yay.
