When there is 2 lenses the same but 1 is macro and the other is not, what actualy is the differance apart from the price. Some thing like Sigma 24-70mm f2.8 IF EX DG HSM compared to Sigma 24-70MM f2.8 EX DG macro?

The things you see when you dont have a camera.
The macro lens is the older version of the two, its not a true macro but it focus's close. The HSM version is the newer one with built in focus motor and the Macro has a screw drive from the camera.
The Macro version won't auto-focus on newer bodies without focus motors built in.
